    INSURANCE

It is your responsibility to understand your insurance policy. You will be responsible for services not covered by your insurance, this includes and is not limited to: co-pays, deductibles, taxes, non-clinical fees, no show fees, and late cancellation fees.

We accept the following insurance providers:
Aetna, America’s PPO, Blue Cross Blue Shield, Cigna, HealthEZ, Health Partners, Mayo Medica, Medica, Medical Assistance, MN Medicare/Medicaid (select providers), Optum, Preferred One, South Country Health Alliance, UCare, United Behavioral Health, and United Health Care

    SELF-PAY

If we are not covered by your insurance, or you are not using insurance, we provide services at an out-of-pocket rate of $110.00 per hour.

  • Fees

If an appointment is canceled less than 24 hours prior to a scheduled session, or you are a no-show for a scheduled appointment, you may be assessed an $80 late cancel/no-show fee that is not billed to insurance. You will be responsible for this fee.

  • Good Faith Estimate

Under the No Surprises Act you have the right to receive a “good faith estimate” explaining how much your medical care will cost. Under this law, providers need to provide an estimate for services to patients who don’t have insurance or are not using insurance. You may ask your provider for this estimate prior to scheduling services. If you receive a bill that is more than $400.00 over your estimate, you may dispute the charges. Please retain your good faith estimate for your records. For more information on these rights visit http://www.cms.gov/nosurprises
